Restraint – Nursing Notes

• Definition:

Restriction of patient’s movement using physical/mechanical devices to ensure safety.

• Indications:

✓ Violent/agitated patient

✓ Prevent self-harm

✓ Prevent removal of tubes/lines

✓ During emergency procedures

• Types:

→ Physical restraint

→ Chemical restraint (as prescribed)

→ Mechanical restraint

• Principles:

✔ Use least restrictive method

✔ Use only when necessary

✔ Doctor’s order required

✔ Explain to patient/family

• Nursing Care:

✓ Check circulation every 15–30 min

✓ Maintain dignity

✓ Provide fluids & toileting

✓ Document reason & time

• Complications:

⚠ Skin injury

⚠ Nerve damage

⚠ Psychological trauma

• Legal/Ethical Points:

→ Follow hospital policy

→ Informed consent if possible

→ Never use as punishment
